Here are the intake forms, please fill them out before your appointment! Deduction spreadsheets are below!

Note, you are only a returning client if I did your 2020 tax return, if you've skipped a year with me, you are new!

What Spreadsheets do I need to fill out?

 1. Did you have acting/artistic jobs that were only on W2s?
Lived or worked in NY: Did you have at least $10,000 in job expenses? (20 if married)
Lived or worked in CA: at least $5000? (10 if married)
Other States: PA 2k-contact me for special instructions, MN 15k, AL AR HI 4k

Then fill out the Large W2 Acting spreadsheet
(Note, if your expenses are smaller, then don't bother. If you have 1099s, see #2!)

2. Did you have at least $1000 on 1099s or undocumented income for acting/artistic jobs (including teaching)?
Then fill out the 1099 Performing Income spreadsheet or the 1099 Behind-the-Scenes spreadsheet
For On-Stage Performers of all stripes - acting, singing, dancing, musicians, comedians, puppeteers, etc! (and teachers of these things)

3. Do you have 1099 or self-employment income from a non-performing business or line of work?
Examples: Baby-sitting, personal training, graphic design, photography, catering, Uber driving, admin work etc!

4. Did you have a large amount (several thousands) of medical expenses, charitable donations, mortgage interest and property taxes or a combination thereof? NJ RESIDENTS please do this sheet for the medical expenses.

6. Did you produce your own show/concert/film? And have income from Fractured Atlas, Kickstarter, or ticket sales?

(Note, This is geared towards one-time things, if you are an ongoing producer use 1099 Behind-the-Scenes)

Fill out as many spreadsheets as apply to you (though do NOT do both #1 and #2, its one or the other!) Also make sure not to double up on expenses, pick whichever spreadsheet the expense most applies to or split the amount.

Married people – you need your own spreadsheets for 1, 2, 3, and 6, you can combine 4 and 5.  

FAQ
Q: Jen, I have mostly W2s and a couple of small 1099s, do I really need to do that whole 1099 spreadsheet?
A: It's up to you!  If you don't really want to spend a ton of time on deductions and don't mind if that means you pay a bit more in taxes, then do the simpler W2 spreadsheet. (#1 vs #2)  

Q: I have a ton of deductions I want to take, but I was only on W2s! Why doesn't Spreadsheet 1 have all those categories?
A: Because W2 expenses are only useful on states, they are worth a lot less.  To save you time and trouble, I created a simplified spreadsheet (#1) that only has the big things on it.  If you still want to go through every little thing, you totally can, just do Spreadsheet 2 instead. 

Q: Should I do both Spreadsheet 1 and 2?
A: Nope!  Please pick one or the other, Spreadsheet 1 is just a simplified version of Spreadsheet 2. 

Q: I don't have Excel, how can I send you the spreadsheet?
A: I can share spreadsheets with you on Google Sheets, or you can use Open Office.  If you have Numbers on your Mac, you can export as an Excel file.  Please do not send me spreadsheets as PDFs or Numbers files, I cannot work with them nearly as well.
